Submit

Special Handling

  • Each specimen container must be labeled with: patient's full name, date of birth, specimen collection date and time, and source that matches the information provided on the order/requisition . Source must be a specific anatomic location on the body.

    The order/requisition must include: patient's full name, date of birth, collection date and time, source (anatomically specific), provider's full name, clinical and billing information.

    Please call for site specific collection information or before submitting any fresh specimen that may require special handling, this includes samples for flow cytometry and cytogenetics.
