Each specimen container must be labeled with: patient's full name, date of birth, specimen collection date and time, and source that matches the information provided on the order/requisition . Source must be a specific anatomic location on the body.
The order/requisition must include: patient's full name, date of birth, collection date and time, source (anatomically specific), provider's full name, clinical and billing information.
