Specimen:

Collect

  • One SST (POST1)
  • One SST (POST2)
  • One SST (PRE)

Submit

  • Three 1 mL (Min:0.5 mL) Serum . Submit Refrigerated. Submit in a Standard Transport Tube.

Special Handling

  • Avoid Repeated Freeze/Thaw Cycles
  • Minimize air exposure
  • Patient should fast a minimum of 10 hours.
    Test should begin with collection of an 8:00 a.m. baseline Cortisol. Cosyntropin is injected IM, and samples drawn at 30 minutes and 60 minutes. Clearly label each sample with collection times.
    For patients receiving therapy with high biotin doses (>5 mg/day), no laboratory test specimen should be collected until at least 8 hours after the last biotin administration.

Interpretive Data

Interpretive data is based on a normal 8:00 am baseline.
Increase in serum cortisol after COSYNTROPIN injection of more than 6 is expected if the adrenal glands are physically active and normally responsive to ACTH.
Absence of such a rise is evidence for primary adrenal insufficiency.
Response may be abnormal after prolonged steroid therapy.

Biotin in specimens taken from patients on high-dose biotin therapy or supplements may intefere with at least one test in this panel and cause inaccurate test results. It is recommended that for patients receiving therapy with high biotin doses (> 5 mg/day), no laboratory test specimen should be taken until at least 8 hours after the last biotin administration.
