There are some symbols and terms that you should be aware of before you start your search. Below is a list of these and an explanation of each.

‡ ABN

This test requires Medical necessity documentation as outlined by HCFA. If ICD-9 diagnostic documentation does not meet Medicare guidelines, a properly executed Advanced Beneficiary Notice is required.

CPT Codes

CPT codes are provided for guidance and are subject to change at any time without notice. CPT coding is based on AMA guidelines and are for information purposes only. CPT coding is the sole responsibility of the billing party. Please direct any questions regarding CPT's to the payor being billed.

¤ Non-FDA Approval

Performance characterization validated by testing laboratory. The U.S. FDA has not approved this test. Results are not intended to be used as sole means of diagnosis or treatment of disease. For Medicare patients, an Advanced Beneficiary Notice is required.

Turn Around Times

Turnaround times provided should be interpreted as the average number of days from the time a specimen is received by Interpath to the time results are completed. Not all tests are performed daily; therefore, the day the specimen is received will affect turn around times in certain situations. Contact Client Services for specific information.
