One 6 mL Pink Top (EDTA)

Two 4 mL Lavender (EDTA)

Submit:

One 6 mL Pink Top (EDTA). Submit Refrigerated.

Two 4 mL Lavender (EDTA). Submit Refrigerated.

Special Handling:

Please label specimen with pre-numbered requisition sticker. Also using a positive identification wristband, label specimen with patient's full name, medical records number (also acceptable: DOB or SS#), date, time, and phlebotomist's initials
Specimen should be less than 72 hours old
If positive, testing will be reflexed to an antibody identification.

Hemolyzed specimens
Specimen not properly labeled
