One 4 mL Lavender (EDTA)
One 6 mL Pink Top (EDTA)

Submit:

4 mL (Min:3 mL) Serum. Submit Refrigerated.
One 4 mL Lavender (EDTA). Submit Refrigerated.
One 6 mL Pink Top (EDTA). Submit Refrigerated.

Special Handling:

Please label specimen with pre-numbered requisition sticker. Also using a positive identification wristband, label specimen with patient's full name, medical records number (also acceptable: DOB or SS#), date, time, and phlebotomist's initials
